The Care Concierge team guides plan members and their families through care options, local services, and new technologies to boost wellness and promote independence within the home.

  • The Care Concierge completes a risk assessment and provides an action plan that includes service recommendations.
  • The team is also available to care recipients and case managers to help navigate services into the home and resolve any customer service requests or complaints.
  • Our Marketplace offering allows the Plan to allocate credits to its care recipients to be used toward predetermined aging-in-place services.

We believe the launch of Lincoln Concierge Care Coordination, in conjunction with Lincoln’s industry-leading long-term care products, will greatly assist policyholders and their families in navigating the process of planning for and finding care, as well as understanding what resources are available to them. By offering these services to our customers before they need long-term care we hope it will help them age in place and lead stronger, healthier lives.
